Sonic Cards - Mrs. Deming's Class

Hi Everyone, we still have a few of the new $5.00 Sonic cards that students would love to sell you. Thank you greatly to the staff who have purchased one or more.

Now we have a great deal on last year Sonic cards: $1.00 for a full Sonic card!  But they expire December 2017. Think about it before you say 'no deal'.  There are 10 food reasons listed on the back of the 2017 card and you have 2 1/2 months to use it and it is only $1.00.

Let us know how many you want at your earliest so we can get them to you ASAP.  Thank you so much! The proceeds of all our fundraising goes to help with classroom supplies and with off-campus trips.

More later on the 5K Run in January!

Thank you to everyone that came out to spirit night at Moe's!

The fundraiser Due date is October 31st. Please contact the front office if you need a packet, or extra order forms.

Follow the PTSA on Facebook. This is a great way to stay up to date and We are listed under Dutch Fork Middle School PTA.

      From the Nurse

Please note that the Health Room is designed to be an emergency station to care for minor illnesses or injuries that occur at school. Injuries or health concerns that occur outside of school should be treated at home or by your healthcare provider. The school nurse cannot diagnose or

prescribe treatment. This is the responsibility of your family physician or health care provider. The nurses in the district were recently reminded that we are not allowed to treat old injuries that happened at home in the health room.  This is per district policy.  The student will always be allowed to call home to notify a parent and ask them to treat or take them to the doctor.

Please be vigilant about keeping your children home if they have any of the following symptoms:

      • Fever and/or temperature of 100 degrees or higher within the past 24 hours

      • Severe nasal discharge that is not clear in color

      • Persistent, uncontrollable cough that is disruptive to the student or class

      • Vomiting and/or diarrhea within the past 12 hours

For more information, please visit our district website under Student Health Services, School Health Room Operations:

      Medication Policy

Please note that the school does not provide any medications.  Medications (including cough drops, over the counter anti-itching creams, etc.) to be taken during school hours must be brought to school and delivered to the school nurse or designated staff in the ORIGINAL container with all labels intact.  Over the counter medications should only be sent to school for a specific condition your child is known to experience.  Whenever possible, medications should be given before or after school.  All medications must be accompanied by parental permission and prescription medication also require written authorization from the prescribing physician or healthcare provider. Herbals, food supplements, alternative medicinal products (i.e. essential oils), and other items that do not have FDA approval also require a written order from a prescribing health care provider.

The Medication Consent Form may be obtained from the district web site at http://www.lexrich5.org (go to Fast Facts, Health Room Services,  Health Room Forms, and then, Medication Consent Form).
