Changing Mode of Transportation

If a student needs to change his/her mode of afternoon transportation for a given school day, please send in a written note or a fax the day before the change (no later than 2:00pm). No emails can be accepted.

Items to include on the correspondence:

• Student Name
• Student Teacher
• Current Date
• Date of Transportation Change
• Parent/Guardian Signature
