Screenings

Screenings assess a child’s overall development as well as health, hearing and vision.  At the completion of the screening the Family Educator provides an overview of the child’s screening results as well as any necessary resources.

Developmental Screening

Your child’s screening supplies information that helps reveal your child’s strengths, uncovers any areas of concern and determines if there are community resources or services that may be useful for your child and your family. 

To accomplish this Ready by Five uses the Ages and Stages Questionnaires, Third Edition (ASQ-3) and the Ages and Stages Questionnaires: Social-Emotional (ASQ:SE).

The ASQ-3 includes five developmental areas:

  • Communication: both what your child understands and what your child can say
  • Gross motor: your child’s ability to use large muscles
  • Fine motor: your child’s hand and finger movement and coordination
  • Problem solving: how your child plays with toys and solves problems
  • Personal-social: your child’s self-help skills and interactions with others

The ASQ:SE-2 is a parent-completed, child-monitoring system for social emotional behaviors. The ASQ:SE-2 is designed to provide information on how a child is developing in seven behavioral areas:

  • Self-regulation
  • Compliance
  • Adaptive functioning
  • Autonomy
  • Affect
  • Social-communication
  • Interaction with people

Health, hearing and vision screening

You will complete a Health Record form, ideally before your screening appointment.  It asks about your pregnancy, your child’s illnesses, teeth, eyes and ears.  This information helps complete a picture of your child’s strengths as well as possible concerns that could affect your child’s development.
