This form is for students with a district issued Chromebook that needs repaired while your student is in virtual learning. Please refer to the District Issued Device Agreement for information on coverage and charges for district devices. Please complete this form and someone will contact you as soon as possible to first try and fix the issue remotely. If we need to issue a loaner to repair the device in person, we will set up an appointment. Please do not come to a school/office without an appointment as there may not be someone there at that time to assist you.
